Entitlement leave applications are where the eligible employee is deceased and the person’s legal personal representative is requesting a payout of long service leave entitlements. Â
Leave applications can only be submitted via the Leave Applications tab within Online Services.
To commence the process
Log into Online Services Â
Go to the Leave Applications tab Â
Click Create New Applications and select the Entitlement leave application type 
Choose the correct employee from the list provided and click Start. If the current employee is not listed due to the levy not being processed, you can choose to ‘Manually add employee’. Â
Review and confirm employee contact details. Update any pre-filled contact details if needed. We will use these details to keep the employees' personal legal representative informed.  Â
Click Confirm Details Â
Complete the following Â
Enter the date of deathÂ
Tick the relevant leave entitlement boxes Â
Upload a certified copy of the official Death Certificate.Â
Complete the declaration and Submit the application. Â
All interactions to access the unclaimed leave of a deceased eligible employee are conducted through an employer and the deceased’s legal personal representative.Â
In accordance with the legislation, only the legal personal representative of a deceased employee may make leave accrual enquiries and lodge a claim through the employee’s last known employer. (Refer Part 5A, Division 3, Section 39CC of the Admin Act.Â
Once an employer receives a written request from the deceased’s legal personal representative, under the legislation the employer must pay any unclaimed accrued leave hours within 30 days. Â
Information related to the leave application can be viewed within the leave application detail page. Simply click on the case number for the leave application to see details and read case messages. Â
The Leave Approved Report is available in the Reports section of Online Services. This report contains relevant information that can help you complete your Claim for Reimbursement.
